Ontario Health Teams

The Government of Ontario is building a connected health care system centered around patients, families and caregivers.

The Ontario Health Team (OHT) model is part of the Ministry of Health’s (MOH) health system transformation strategy that will enable patients, families, communities and providers to work together to achieve better population health outcomes.
