CCHOHT 2025-26 Operating Plan Engagement Session (In-person)

CCHOHT has officially begun the development of its 2025–26 Annual Operating Plan. This year, Ontario Health has identified three priority areas for all Ontario Health Teams:
- Primary Care Access, Attachment and Enablement
- Integrated Clinical Priorities
- OHT Capacity Building
CCHOHT is excited to host an in-person engagement session for members of the public interested in health system changes in our community, primary care providers, and its partner organizations to gather input on CCHOHT’s 2025-26 Annual Operating Plan based on this year’s priorities. Parking is free and food (pizza) will be provided.

Focus Questions
This session will feature discussion tables for participants to offer their input on three focus questions related to each priority area. To make this session an interactive and productive experience for everyone involved, we encourage participants to read these questions in advance and brainstorm ideas prior to our meeting.
Priority #1: Primary Care Access, Attachment and Enablement
- Please tell us what you would like the CCHOHT to do better to provide information and helpful alternatives to people using the Emergency Department for non-urgent needs (e.g., colds, flu, struggles with housing/food insecurity, mental health concerns, etc.).
- What ideas or positive experiences can you share on how CCHOHT can improve access to timely Primary Care when needed?
- How do you see your involvement in advancing a Primary Care Network (PCN), based on Ontario Health’s vision for PCNs as defined below? (You can refer to the Guidance Document on Primary Care Networks in Ontario for more information on PCNs).

Priority #2: Integrated Clinical Priorities
- What ideas do you have for impact in raising awareness through the CCHOHT on the importance of cancer screening promotion? What could outreach look like to bring information to newcomers and marginalized populations to support increasing screening rates?
- How can the CCHOHT better integrate community supports into family doctor/primary care offices?
- What are the greatest opportunities to support frail seniors/older adults and other at risk patients in our communities to prevent potentially unnecessary hospitalizations?
Priority #3: OHT Capacity Building
- What would meaningful involvement in the identification and implementation of new CCHOHT programs/supports look like to you?
- What training and tools could the CCHOHT provide to help you participate in health system planning in an impactful way?
- What engagement formats work best for you and how can the CCHOHT improve our communication and interaction with you? (E.g., social media, newsletters, website, town halls, etc.)
