CCHOHT 2025-26 Operating Plan Virtual Engagement: Priority #1 (Session 1 of 2)

This is virtual session 1 of 2 for Priority #1: Primary Care Access, Attachment and Enablement. You only need to attend one virtual session for each priority.

This session will focus on obtaining input on how CCHOHT can make progress and gather recommendations for improvements for its 2025-26 Operating Plan.

Focus Questions

Discussion will be centered around three focus questions related to the priority area. To make this session an interactive and productive experience for everyone involved, we encourage participants to read these questions and brainstorm ideas in advance of our meeting.

1. Please tell us what you would like the CCHOHT to do better to provide information and helpful alternatives to people using the Emergency Department for non-urgent needs (e.g., colds, flu, struggles with housing/food insecurity, mental health concerns, etc.).
   
   
2. What ideas or positive experiences can you share on how CCHOHT can improve access to timely Primary Care when needed?
   
   
3. How do you see your involvement in advancing a Primary Care Network (PCN), based on Ontario Health’s vision for PCNs as defined below? (You can refer to the Guidance Document on Primary Care Networks in Ontario for more information on PCNs).
