CCHOHT has established a Digital and Analytics Working Group, which will identify opportunities for digital health solutions to support effective healthcare delivery, ongoing quality and performance improvement, and better patient experience. The working group will have a commitment to develop solutions to share information and to adopt/provide digital options for decision support, operational insights, and population health management.

Activities Completed to Date

Harmonized Information Management Plan (HIMP): Submission to Ontario Health as a tool and template to ensure emphasis on data governance and data stewardship for CCHOHT to meet the goals of integrated care while protecting privacy

Universal Data Sharing Agreement: Endorsed by CCHOHT Collaborative Steering Committee for technology implementation across organizations and partners to enable seamless integration of patient health information

Digital Asset Inventory and Virtual Care Maturity Assessment across the Halton Region: Completed assessments to understand gaps and opportunities

Collaborative Quality Improvement Plan (cQIP): Submission to Ontario Health and on-going indicator monitoring and identification of gaps and opportunities for the following indicators:

  • Rates of emergency department visits as first point of contact for mental health and addictions–related care
  • Percentage of alternate level of care days
  • Percentage of screening eligible patients up-to-date with Papanicolaou (Pap) tests
  • Percentage of screen-eligible patients up-to-date with a mammogram
  • Percentage of screen-eligible patients up-to-date with colorectal screening

Remote Care Management: Year two sustainment and expansion funding approved

Surgical Transitions: Year one funding approved

Online Appointment Booking: Year three sustainment and expansion funding approved in collaboration with the Mississauga Ontario Health Team

Upcoming key activities

Spread and scale of Remote Care Management to Palliative Care, Diabetes and Mental Health and Addictions patients/clients to promote self-management, early discharge from hospital and avoidance of emergency department visits. The program enables patients to manage symptoms at home with technology and nursing support.

Launch of the Virtual Total Joint Recovery Program which will allow patients to access resources to aid in surgical preparation and recovery, answer questions about pain, wound care and connecting with the care team to support patients at home during the early days of recovery.
